asymmetric and patchy involvement. dactylitis (sausage fingers) and pencil-in-cup deformity on x ray. fewer than 1/3 of pts with psoriasis
ankylosing spondylitis
spine and sacroiliac joint disease. see stiff spine from joint fusion (bamboo spine), uveitis, and aortic regurg
What infections may be followed by reactive arthrits?
GI (shigella, salmonella, yersinia, campylobacter) or chlamydia
What do you see in sarcoidosis?
widespread non-caseating granulomas and elevated serum ACE levels
What are some symptoms in sarcoid?
restrictive lung disease, erythema nodosum, lupus pernio, Bell palsy, epitheliod granulomas with asteroid bodies, uveitis, and hyper calcemia (d/t vitamin D activation in macrophates)
Polymyalgia rheumatica. what is it associated with?
pain and stiffness in shoulders and hips, often with fever, malaise, and weight loss. no weakness. associated with giant cell arteritis.
Findings and treatments for polymyalgia rheumatica
incr. ESR, incr CRP, normal CK. give low-dose corticosteroids
With what other problem is myasthenia gravis often associated?
thymoma

desmin positive rhabdomyoblast. most common site is the head and neck. seen in the vagina of young girls
