What is Septic Arthritis?
A
Infection within a joint - emergency as the infection can quickly begin to destroy the joint and cause systemic illness.

Clinical Features of Septic Arthritis (3).
A
- Hot, red and swollen painful joint.
- Stiffness and reduced range of motion.
- Systemic symptoms.
* only a single joint - often the knee, with rapid onset.

Aetiology of Septic Arthritis (5).
A
- S. aureus (commonest).
- N. gonorrhoea (sexually active).
- Group A Streptococci (S. pyogenes).
- H. influenzae.
- E. coli.

Differential Diagnoses of Septic Arthritis (4).
A
- Gout.
- Pseudogout.
- Reactive Arthritis.
- Haemarthrosis (bleeding into joint).

Diagnosis of Septic Arthritis (4).
A
KOCHER CRITERIA :
1. Fever > 38.5.
2. Non-Weight Bearing.
3. Raised ESR.
4. Raised WCC.

Investigations of Septic Arthritis.
A
SYNOVIAL FLUID SAMPLING : Aspirate Joint - send sample (may contain pus) for MC&S, gram staining, crystal microscopy.

Management of Septic Arthritis (4).
A
- Low Threshold (examine joint fluid).
- Local Hot Joint Policy - which team admits, what antibiotics to use, and how long.
- Empirical IV Antibiotics until Sensitivities are known.
- Needle Aspiration +/- Arthroscopic Lavage.

Antibiotic Therapy in Septic Arthritis (4).
A
- 3-6 weeks.
- Local guidelines.
- 1st Line - Flucloxacillin + Rifampicin.
- Penicillin Allergy, MRSA, Prosthetic Joint - Vancomycin + Rifampicin or Clindamycin.
