What is Fibromyalgia?
Syndrome characterised by widespread pain throughout the body with tender points at specific anatomical sites.
Epidemiology of Fibromyalgia.
- Women 5:1 Men.
2. Age : 30-50.
Clinical Features of Fibromyalgia (4).
- Chronic Pain at Multiple Sites.
- Lethargy.
- Cognitive Impairment : FibroFog.
- Sleep Disturbance, Headaches, Dizziness.
Diagnosis of Fibromyalgia.
Clinical - tenderness at 11 of 18 joints in the body lasting at least 3 months, front, back, right and left, above and below diaphragm.
Management of Fibromyalgia.
- Information Giving.
- Aerobic Exercise.
- CBT.
- Neuropathic Pain Relief Medications.
What is the difference between Acute and Chronic Pain?
Acute - new onset.
Chronic - 3+ months.
What are the 2 types of chronic pain?
- Chronic Primary Pain - no underlying condition adequately explains the pain.
- Chronic Secondary Pain - underlying condition explains the pain.
What medications should not be started on for Chronic Primary Pain? (5)
All forms of analgesia :
- Paracetamol.
- NSAIDs.
- Opiates.
- Pregabalin.
- Gabapentin.
What medications should be used to treat Chronic Secondary Pain? (3)
- Oral Paracetamol and Topical NSAIDs.
- Oral NSAIDs (with PPI).
- Opiates e.g. Codeine.
