Crystal Arthritis - Pseudogout Flashcards
1
Q
What is Pseudogout?
A
Crystal arthropathy caused by Calcium Pyrophosphate Dihydrate crystals.
2
Q
Risk Factors of Pseudogout (5).
A
- Increasing Age.
- Haemochromatosis.
- Hyperparathyroidism.
- Low Magnesium + Low Phosphate.
- Acromegaly + Wilson’s Disease.
3
Q
Aetiology of Pseudogout.
A
Calcium Pyrophosphate crystals are deposited in the joint - also known as Chondrocalcinosis.
4
Q
Clinical Presentation of Pseudogout (3).
A
- Stereotypical Patient : Older Adult with hot, swollen, stiff, painful knee.
- Asymptomatic (+ Incidental X-Ray Finding).
- Chronic Condition affecting multiple joints e.g. shoulders, wrists, hips.
5
Q
Diagnosis of Pseudogout (2).
A
- EXCLUDE SEPTIC ARTHRITIS.
2. Joint Aspiration - Synovial Fluid.
6
Q
What will Aspirated Fluid show in Pseudogout? (4)
A
- No bacterial growth.
- Calcium Pyrophosphate Crystals.
- Rhomboid Shaped Crystals.
- Weakly Positive Birefringent of Polarised Light.
7
Q
X-Ray Changes in Pseudogout (5).
A
- LOSS.
- Chondrocalcinosis - thin white line in the middle of the joint space caused by Calcium deposition : pathognomonic of pseudogout.
8
Q
Management of Pseudogout (3).
A
- Chronic - No Management.
- Symptomatic - NSAIDs, Colchicine, Joint Aspiration, Steroid Injections, Oral Steroids.
- Severe - Joint Washout (Arthrocentesis).
