How does organ dysfunction occur in sepsis?
bacteria enters the blood and the capillaries get leaky leading to organ dysfunction
What is septic shock?
hypotension despite fluid resucitation and vasopressors, lactate >2
What is the mortality of septic shock?
40%
What do you Take 3 and Give 3 of?
Take: blood, urine, lactate
Give: fluids, antibiotics, oxygen
What is the timeframe that the sepsis 6 must be delivered by?
1 hour
What sats of oxygen should be delivered? What about for COPD?
94-98%
88-92% for COPD
How many blood cultures should be taken?
3
How many mls of fluids should be given?
500mls bolus of 0.9% saline
What MAP should be aimed for?
> 65mmHg
What would a high lactate suggest?
hypoperfusion and hypotension
What should be given if still a low BP/lactate >4 after fluids?
vasopressors - noradrenaline
What do you do if noradrenaline is not working?
refer to ICU for vasopressin
What is the target urine output?
0.5ml/kg/hour
