A

Describes the activities taking place on a licensed premises, and how the premises will operate

  1. The type of premises (e.g. supermarket, public house)
  2. The licensable activities taking place (i.e. sale of alcohol by retail)
  3. The name and address of the DPS, off the premises or both (for premises that sell alcohol only)

  5. Any other times the premise will be open
