CHA2DS2-VASc for PO anticoag indication in the setting of Afib
CHF HTN Age >75 (2pts) DM Hx stroke (2pts) Vascular Disease (PAD/MI) Age >65 (1pt) Sex Category (+1 for female)
0= no therapy 1= none, aspirin, or PO anticoag 2+ = PO anticoag
Well’s Criterion (>4 = Likely)
DVT clx or alternate dx 100), recent surg (4wks), immobile (1.5 pts)
Hemoptysis, cancer (1pt)
CKD Staging (GFR)
Stage I: GFR >90 (no tx)
Stage II: GFR 60-89 : tx comorbidities, ACEi/ARBs
Stage III: 30-59 : tx comorbidities, ACEi/ARBs
Stage IV: 15-29: consideration for transplant
Stage V:
CHF NYHAA Staging (Clx)
Class I: No symptoms
Class II: Symptoms with being active, none at rest
Class III: Symptoms daily activities (walk to mailbox)
Class IV: symptoms at rest
Light’s Criteria (pleural fluid, transudate vs exudate)
LDH: (fluid: serum): >.6
LDH: fluid >2/3 ULN serum
protein (fluid: serum): >.5
Remember: Exudate: PNA, malig, PE
Transudate: CHF, nephrotic syndrome, cirrhosis
JONES Criteria for Rheumatic Fever
*requires 2 major or 1 major 2 minor + hx of strep pyogenes
Migratory Joint Arthritis Carditis Subcutaneous nodules Erythema Marginatum Sydenhams Chorea
Minor: ESR, CRP, ASO, arthralgias, prolonged PR, Leukocytosis
Dukes criteria (infective endocarditis)
Bacteremia
New regurg
Vegetation on echo
IVDU, hx of endocarditis, multifocal PNA, splinter hemorrhages, janeway lesions, fever, roth spots, Osler nodes, RF+, glomerulonephritis
