pathology
too much dopamine
diagnostic criteria
1 or more of following:
- Delusions - bizarre delusions or thought control
- Control hallucinations - thoughts can be heard aloud/3rd person or a running commentary
2 or more of following:
- Other hallucinations
- Thought disorder
- Catatonia
- Negative symptoms (apathy, poverty of thought and speech, blunting of affect, social isolation, poor self care)
- Change in personal behaviour - loss of interest, social withdrawal
management
bio - atypical first line, BZD for anxiety, procyclidine for EPSE. Depots if needed. Clozapine last resort (agranulocytosis)
Psycho - family therapy, compliance therapy, psychoeducation
Social - social work and housing if needed
define schizoaffective disorder
schizophrenic and mood (depressive or manic) symptoms
define delusional disorder. when is typical onset
delusion for at least 3 months with only symptom. Typical onset is middle age.
drugs that can cause psychosis
alcohol, amphetamine, cocaine, inhalants, corticosteroids, anticholinergics, anti-parkinsons drugs
differentials for schizophrenia
manic episode with psychosis, depression with psychosis, delirium, substance induced, temporal lobe epilepsy
