difference between panic and anxiety disorders
panic - unpredictable, not restricted to specific situations
anxiety - well defined situations, predictable
define social anxiety disorder
fear of negative evaluation by others, avoidance of feared situations and use of safety behaviours
define specific phobias
restricted to specific objects or situations
Diagnostic criteria of OCD
2 weeks of intrusive thoughts and compulsions to fend them off. Acknowledged as coming from pts own mind.
Diagnostic criteria of GAD
6 months of Autonomic arousal + 3 others of:
WATCHERS - Worry, Anxiety, Tension in muscles, Conc, Hyperarousal, Energy, Restlessness, Sleep
PTSD diagnostic criteria
TRAUMA - Traumatic event (within 6 months), Re-experience, Avoidance, Unable to function/disinterest in usual activities, Month or more of symptoms, Arousal increased
manage panic disorder
SSRIs +/- BZDs, CBT. Emergencies use BZD. ST BZDs are diazepam and lorazepam
Psychoeducation and CBT
manage phobias
graded exposure
manage OCD
SSRIs, TCAs, CBT (graded exposure), ECT if suicidal
manage GAD
Buspirone (ST), pregabalin, antidepressants, CBT, psychoeducation
manage PTSD
SSRIs, nitrazepam (insomnia), BZD (hyperarousal), CBT, EMDR
Differentials for anxiety disorders
CHAP - Cushings, hyperthyroidism, Anemia, Pheochromocytoma
