A
  • Safe speed
  • Navigation lights on
  • Start fog signal
    (one prolonged no more than 2min interval)
  • Post lookout on the bow with radio and report when on station
  • Put engine room on standby
    (shut watertight doors)
  • Clear anchors
    (on the brake)
  • Security call / VTS for expected traffic

Steering Failure

A
  • Order helmsperson to switch to backup pump
  • If backup pump failure go to NFU
  • Call engine room
    (check on status)
  • Safe Speed
  • If down to one pump, or no pump - man emergency steering station

If NO steering…
- Radio “Pan-Pan, Pan-Pan, Pan-Pan”
- Initiate emergency steering
- Take way off / navigate to safe area
(wind, current, bottom characteristics using thruster, transverse thrust, and kicks ahead)
- Standby anchors
(consider dredging to maneuver, chain 1.5x to 2x depth)
- Call VTS / Tug assistance
- Not under command lights/shapes
(two all-around red lights, sidelights/sternlight while making way, two black balls)
- Order Mate to modify AIS status
- Sound signal if in reduced visibility
(one prolonged, two short at no more than 2 min interval)

Gyro Failure

A
  • manifest as “locked” gyro heading
  • Know/note magnetic compass heading/course and have helmsperson steer by magnetic
  • Call engine room for electrician to remedy
  • Safe speed
  • Switch Radars to head-up relative motion and relative vectors
  • Use EBL and trails to track targets
  • Call VTS to report Gyro and ARPA failure
  • Radio “securite, securite, securite”
  • Order Mate to check on repair progress

  • Use rudder early to take way off (rudder cycling) and head to safe water

    (prepare for dredging, chain length 1.5x to 2x depth)
  • Not under command lights/shapes
    (two all-around red lights, sidelights/sternlight while making way, two black balls)
  • Order Mate to modify AIS status
  • Sound signal if in reduced visibility
    (one prolonged, two short at no more than 2 min interval)
  • Check bottom characteristic (type and slope)
  • Evaluate wind and current

  • Check tides for refloating, bottom characteristic, tug assistance

  • Assess and maneuver to assist firefighting effort

  • Call VTS / assistance
  • Not under command lights/shapes

  • Dismiss extraneous person from the bridge
  • Order crew to put their cell phone away
  • Order Mate to notify crew that the bridge is closed to all communication other than that relevant to the safe navigation of the vessel and urgent ship business
  • Order Mate to answer ship phone and reply with “standby” or “will call back”

  • Order “Rudder Midship,” then give new rudder command
  • If orders are repeatedly failed to be executed then order Mate to call a relief for the helmsperson

Anchoring

A
  • Check bottom characteristics, wind, current, depth
  • Chain length (5x depth)
  • Approach, placement, veering out, fetching up
  • Deep water procedure if applicable (greater than 100 ft depth)
  • Call VTS when at anchor and how many shots in the water
  • Change lights/day shapes (two all-around white lights, stern lower than bow, deck lights, black ball at bow)
  • Order mate to change AIS status
  • Sound signals if in reduced visibility (5 sec ringing of a bell in forepart followed by 5 sec sounding of a gong in after part at no more than 1min interval…optional one short-one prolonged-one short whistle for approaching vessels)
