1 - Asymptomatic with slight nuchal rigidity
2 - Severe headache, stiff neck, no neuro deficit
3 - Drowsy or confused, Mild focal nerve deficit
4 - Stuporous, moderate to severe hemiparesis
5 - Coma, decerebrate posturing
