Simple: one point w/ no compromise to vasc
Closed loop: 2 points w/ no compromise to vasc
Strangulated: compromised blood supply
Presentation of obstruction
Abdo pain
- colicky
- location depends on gut region
- constant pain suggests strangulation or impending perf
Distension
Vomiting
- early in high obstruction
- late or never in low obstruction
Absolute constipation
Ix in obstruction
Abdo xrays ERECT: identifies obstruction
Bloods: FBC, Urea, U+Es, amylase, VBG, G+S
Medical Mx of obstruction
RESUS: NBM + IV fluids
NGT to decompress UGIT
Catheterize
Therapy: analagesia + abx if strang. or perf.
Monitor
Surgical Mx of obstruction
Indications: anything but simple obstruction or if medical mx has failed
