minimum 4-6 hourly monitoring, inform RN who must assess pt, and then RN should decide whether to inc frequency of monitoring or escalate care
what would you do for news score of 3 in single parameter
minimum 1 hourly monitoring. RN to inform medcal team carinf for pt who will review and decide if escalation needed
what would you do for news score of 5 or more
min 1 hourly. RN to immediately inform medical team, RN request urgent assessment by clinican/ team with core competencies in care of acutely ill pt. provide clinical care in environment with monitoring facilities
what would you do for news score of 7 or more
continuous monitoring of vital signs. RN to infrorm medical team caring for pt immediately, should be atleast specialist registrar level. emergency assessment by a team with critical care competencies including practicioner with advanced airway management skills. consider transfer to level 2/ 3 clinical care,

hi is this the oncall registrar?
my name is Lesly Aju and I am a 2nd year medical student on placement in acute medical ward.
Situation
I am calling regarding Mr Samuel wilson, who I would like you to review/ take over care/ give me some advice about because— I am concerned that (parameter is 99 and other parameter is 98, and NEWS is bad
Background
Mr Wilson is a 51 year old male who was admitted today to the acute medical ward with chest pain worse on breathing in.
since admission they have had— (they have not had any investigations done)
Mr Wilsons condition has changed in the last (XX minutes).
his last observations at X o’clock was 0.
pt normal condition is (drowsy/ confused/ pain free)
assessment:
I think the probelm is (XXX) and I have (eg given O2/ analgesia/ stopped infusion)
or
I am not sure what the problem is, but the Pt is deteriorating/
I don’t know what is wrong but I am worred.
Recommendation
I need you to XXX (eg please come and review the pt when your next free/ in the next XX min/ immediately)
AND- is there anything I need to do in the meantime?
