How do you set up your airway step?

A
  • 2 different laryngoscope blades and handles - check light on each
  • 2 ETT sizes appropriate for gender - adule male ~8.0mm, adult female ~7.0mm (place stylet through tube not extending past murphy eye and shape into “smily face”, check cuff by filling with 10cc air
  • 2 different sizes of oral airways
  • 1 tongue blade
  • Ambu bag present
  • LMAs present
  • Bougie present

How do you set up your monitor?

A
  • Alarms on and audible - set up appropriate parameters
  • Monitoring equipment present - EKG leads, pulse ox, BP cuff, ETCO2, nerve stimulator, temp probe

First thing you do for machine check

A

Turn power outlet on and verify machine is plugged into a red power outlet

2nd thing you do for a machine check because it takes the longest

A
  • Locate and expose the O2 analyzer to room air for 3min
  • then calabrate the O2 analyzer with room air
  • Reinsert O2 analyzer into machine and expose it to 100% O2 for 3 min (O2 doesn’t need to be turned up for this step - remember 100mL/min is always running through when machine is on)
  • Then Calabrate to 100% after the 3 min exposure

After you expose the O2 analyzer to RA for the 3min, what can you be doing in the meantime?

A

Locate cylinder wrench to check back-up tanks at the back of the machine
- We want >500PSI in O2 (change if 500 or lower) full O2 is 1900PSI

We can also check the pipeline pressure on the front of the machine
- We want them all between 45-55PSI

How do you perform the breathing circuit pressure check?

A
  • Turn off all flowmeters and close the APL
  • Occlude distal end of the breathing circuit with the knotch on the side of the machine
  • Pressurize the circuit to 30-40cmH2O with the O2 flush valve
  • Squeeze the bag to make sure theres no leak with extra pressure
  • open and close each vaporizer to make sure no leak
  • Open APL and squeeze the reservoir bag and look at the scavenging system to make sure that is working (the clear scavenging bag should inflate and deflate as the air goes into that system)
  • while you are looking on the side of the machine, look at the CO2 absorbant and make sure its not exhausted

How do you check Vent function?

A
  • Place reservior bag on the end of the circuit to make a fake lung (keep APL open)
  • turn O2 flow up to 6-10L/min
  • Turn on vent mode and watch bellows descend on inspiration and ascend on expiration (feel the bag inflate and deflate)
  • Check flutter valves movement (if stuck, biomed needs to evaluate)
  • When done, turn back to manual, put bag back, put the mask on the circuit and turn off O2 flows

A
  • Prepare drugs for induction and emergencies
  • label the syringe immediately after drawing up med (concentration, date, initials) check each med 3 times
