What are intrinsic factors?
Advanced age, previous falls, muscle weakness, gait problems, poor vision
What are extrinsic factors?
Poor stair design, lack of handrails, lack of grab bars, dim lighting, tripping hazards, slippery surfaces
What are some alternatives to restraints?
Rule out causes, reduce stimulation, distract patient, use bed alarm
What are the rules regarding physical restraints?
Physician order needed with duration, patient monitored at least every hour
What documentation is needed for restraints?
Date and time, type of restraint, alternatives attempted and results, family notification, frequency of assessment, findings, regular intervals, and nursing interventions
How do you apply restraints?
Least restrictive with greatest degree of mobility, pad bony prominences, ensure 2 fingers can be placed between restraint, use quick release knot on bed frame not side rail, remove restraints every 2 hours
What does RACE stand for?
Rescue anyone in immediate danger
Activate the fire code system and notify appropriate person
Confine the fire by closing doors and windows
Evacuate patients and others to a safe area
What does PASS stand for?
Pull
Aim
Squeeze
Sweep
