What are consequences for lack of perineal care?
Bacteria growth, physical and psychological discomfort, skin breakdown, and offensive odors
What are implementations for a bed bath?
Ensure water is comfortable temperature, place items on bedside table, ensure privacy, mirror/light/hot water for patients wanting to shave, assist patients who cannot bathe themselves
How are disposable bath wipes used?
Warmed in microwave or heater until use, each part of patients body use fresh cloth, no rinsing required, air dry for 30 seconds
What is the purpose of CHG bath?
Reduce colonization of skin pathogens, reduce HIA incidence, decrease infection resistant microorganism spread
What is the purpose of skin barrier creams?
Protect skin from excessive moisture exposure such as urine or feces
How is vaginal care provided?
Plain soap and water, from front to back, least contaminated to most contaminated
How is male perineal care provided?
Clean tip of penis first, circular motion from meatus outward, wash shaft in downward strokes, wash and rinse scrotum
How is oral care provided?
Brush and floss teeth twice a day after eating, clean tongue with toothbrush, provide mouthwash
How should dentures be cared for?
Daily soaking and brushing, brush patient gums and tongue, rinse mouth before replacing dentures, store in cold water when not in use
How are hearing aids cared for?
Check batteries routinely, clean daily with mild soap and water
How is an unoccupied bed made?
Loosen linen as move around bed, fold reusable linens and hang over clean chair, snugly roll soiled linen inside bottom sheet and hold away from body and place directly into hamper, push sheet open to center of mattress and pull sheet taut from second corners, move to other side and secure bottom linens, pull topsheet tight and secure over corners and tuck under mattress, fold upper 6 inches of top sheet to create a cuff
How is an occupied bed made?
Lower side rail nearest you, place blanket over patient, reach under and remove top linen, fold over clean chair, assist patient to turn toward opposite side of bed and reposition pillow under head, loosen bottom linens from head, foot, and side of bed, roll soiled linens as close to patient as possible, use clean linen and make near side of bed, push sheet taut and position under old linens, raise side rail and assist patient to roll toward clean linen, repeat on other side, assist patient to turn back to center of bed
