- NTFY Div. by tel. AND fax & IMMED Replace
- RT-2 - If contam’d, = Noted on RT-2 & faxed to R&D; p/u by SOC
- CJ & ORJ entries
- When new belt rec’d = CJ & ORJ entries
- OOS Life Safety Equip. Rpt. (SCF-2) fwd’d thru chain to C. Of
Safety
- BC ON DUTY investigates - A statement to this effect required as
part of BC endorsement.
- Copy of RT-2 & SCF-2 faxed to R&D “& then attached to OOS
belt
