Mech of action
Nicotinic ACh R antagonist > competes with ACh at neuromuscular junction and prevents binding thus inhibiting skeletal muscle depolarization.
Class
Non-depolarizing neuromuscular blocker
Pharmakokinetics
Onset- 60-70 seconds
Peak- 1-3 minutes
Duration- 25-40 minutes
HL- 14-18 minutes
Indicatons
Need for temporary paralysis to facilitate endotracheal intubation
Need for continued paralysis following endotracheal intubation
Contraindications
Hypersensitivity
Side effects
Tachycardia, hypertension, hypotension, bronchospasm(rarely)
Dose
Adult- 0.6-1.2 mg/kg IV, IO
Pedi- 0.6 mg/kg IV, IO
Precautions
Presence of neuromuscular diseases may be associated with prolonged paralysis
Drug - Drug
Potentiated by lidocaine, prcainamide, B blockers, K-sparing diuretics, mag sulfate, and other neuromuscular blockers including succinylcholine
