Mech of action
Opiate R agonist > hyperpolarization of internuerons and decreased release of neurotransmitters responsible for the transmission of pain impulses to the thalamus and cerebral cortex

Narcotic analgesic
Pharmakokinetics
Onset- Immediate
Peak- 3-5minutes IV
Duration- 30-60 minutes
HL- 6-8 hours
Indicatons
- Severe pain
- Adjunct for RSI
Contraindications
- respiratory depression or insufficiency when considering for pain management
- Uncorrected hypotension
- Known hypersensitivity
Side effects
Bradycardia, respiratory depression, apnea, chest wall rigidity
Dose
Adult- 1mcg/kg slow IV, IO, IN max single dose 100mcg, repeat after 10 min
Adult >65 0.5mcg/kg max 50mcg repeat in 5 minutes
Pedi- 1-2 mcg/kg slow IV, IO, IN
Precautions
Caution in patients with kidney and liver dysfunction
Drug - Drug
- Additive effects w/CNS depressants
- Should not be given with patients using MAOI in the last 14 days - may cause Seratonin Syndrome
