RLA Levels of Cognitive Functioning Flashcards
RLA Level of Cognitive Functioning
How many levels are there? What are their respective labels?
There are 8 levels.
I - No response II - Generalized response III - Localized response IV - Confused-Agitated V - Confused-Inappropriate VI - Confused-Appropriate VII - Automatic-Approprite VIII - Purposeful-Appropriate
RLA Level of Cognitive Functioning
Patient appears to be in a deep sleep and is completely unresponsive to any stimuli
I - No response
RLA Level of Cognitive Functioning
Patient reacts inconsistently and non-purposefully to stimuli in a nonspecific manner.
II - Generalized response
RLA Level of Cognitive Functioning
Responses are limited and often the same regardless of stimulus presented.
II - Generalized response
RLA Level of Cognitive Functioning
Responses may be physiological changes, gross body movements, and/or vocalization.
II - Generalized response
RLA Level of Cognitive Functioning
Patient reacts specifically but inconsistently to stimuli.
III - Localized response
RLA Level of Cognitive Functioning
Responses are directly related to the type of stimulus presented.
III - Localized response
RLA Level of Cognitive Functioning
May follow simple commands such as closing eyes or squeezing hand in an inconsistent, delayed manner.
III - Localized response
RLA Level of Cognitive Functioning
Patient is in a heightened state of activity.
IV - Confused-Agitated
RLA Level of Cognitive Functioning
Patient lacks short- and long-term recall.
IV - Confused-Agitated
RLA Level of Cognitive Functioning
Gross attention to environment is very brief; selective attention is often nonexistent.
IV - Confused-Agitated
RLA Level of Cognitive Functioning
Verbalizations are frequent and are incoherent and/or inappropriate to the environment; confabulation may be present.
IV - Confused-Agitated; OR
V - Confused-Inappropriate
Worded in level V as: Verbalization is often inappropriate and confabulatory.
RLA Level of Cognitive Functioning
Does not discriminate among persons or objects; is unable to cooperate directly with treatment efforts.
IV - Confused-Agitated
RLA Level of Cognitive Functioning
Behavior is bizarre and non-purposeful relative to immediate environment.
IV - Confused-Agitated
RLA Level of Cognitive Functioning
Patient is able to respond to simple commands fairly consistently. However, with increased complexity of commands or lack of any external structure, responses are non-purposeful, random, or fragmented.
V - Confused-Inappropriate
RLA Level of Cognitive Functioning
Demonstrates gross attention to the environment but is highly distractible and lacks ability to focus attention on a specific task.
V - Confused-Inappropriate
RLA Level of Cognitive Functioning
With structure, may be able to converse on a social automatic level for short periods of time.
V - Confused-Inappropriate
RLA Level of Cognitive Functioning
Memory is severely impaired; often shows inappropriate use of objects; may perform previously learned tasks with structure but is unable to learn new information.
V - Confused-Inappropriate
RLA Level of Cognitive Functioning
Patient shows goal-directed behavior but is dependent on external input or direction
VI - Confused-Appropriate
RLA Level of Cognitive Functioning
Follows simple directions consistently and shows carryover for relearned tasks such as self-care.
VI - Confused-Appropriate
RLA Level of Cognitive Functioning
Responses may be incorrect due to memory problems, but they are appropriate to the situation.
VI - Confused-Appropriate
RLA Level of Cognitive Functioning
Past memories show more depth and detail than recent memory.
VI - Confused-Appropriate
RLA Level of Cognitive Functioning
Patient appears appropriate and oriented within the hospital and home settings; goes through daily routine automatically, but frequently robot-like.
VII - Automatic-Appropriate
RLA Level of Cognitive Functioning
Patient shows minimal to no confusion and has shallow recall of activities.
VII - Automatic-Appropriate
RLA Level of Cognitive Functioning
Shows carryover for new learning but at a decreased rate.
VII - Automatic-Appropriate
RLA Level of Cognitive Functioning
With structure is able to initiate social or recreational activities; judgment remains impaired.
VII - Automatic-Appropriate
RLA Level of Cognitive Functioning
Patient is able to recall and integrate past and recent events and is aware of and responsive to environment.
VIII - Purposeful-Appropriate
RLA Level of Cognitive Functioning
Shows carryover for new learning and needs no supervision once activities are learned.
VIII - Purposeful-Appropriate
RLA Level of Cognitive Functioning
May continue to show a decreased ability relative to pre-morbid abilities, abstract reasoning, tolerance for stress, and judgment in emergencies or unusual circumstances.
VIII - Purposeful-Appropriate
Level IX
Purposeful-Appropriate; Stand-by assistance on request
Level X
Purposeful-Approproate; Modified independent
