- Rheumatoid nodules (on extensor surfaces & pressure points)
- Ocular (episcleritis, scleritis & scleromalacia perforans)
- Pulmonary (pleural disease, ILD, bronchiolitis obliterans, pulmonary nodules)
- Cardiac (pericarditis, myocarditis, conduction defects)
- Hematologic (IDA, Felty syndrome [splenomegaly, neutropenia & leg ulcers], high or low platelets, Non-Hodgkin lymphoma)
- Neurologic (atlantoaxial instability/subluxation), peripheral neuropathy, ischemia associated with vasculitis
- Rheumatoid vasculitis (leukocytoclastic, cutaneous ulcers)

- path: multisystem noncaseating granulomatous disorder (T helper & inflammatory cells) of unknown etiology
- pres: fever, fatigue, weight loss, polyarthritis, cough, hemoptysis, DOE, Bell palsy, peripheral neuropathy, subQ nodules, lupus pernio, CHF, dysrythmias, uveitis, conjunctivitis, LAD
