Rheumatic fever
Inflammatory disease following a beta hemolytic streptococcal pharyngitis, re: strep throat. Get proliferative inflammation on the heart, joints and subq tissue. Symptoms start 1-5 weeks post infection, and peak incidence is at 5-15 years old.
Most often involved valve affected with Rheumatic Heart disease
Mitral valve- 75-80% of the time.
Lab tests for rheumatic heart disease
none are diagnostic. Can see leukocytosis and anemia in a CBC indicating chronic disease, and can have an elevated ESR and C-reactive protein.
Jones criteria needed
2 major OR one major and two minor
Major Jones Criteria
Carditis, erythema marginatum, subcutaneous nodules, chorea and arthritis
Minor Jones criteria
fever, polyarthralgias, reversible prolongation of PRI, rapid SED rate, hx of rheumatic fever, evidence of Beta-hemolytic streptococcal infection
