A
- fever, chills

A
- Rashes or Itching (pruritus)

- changes in moisture or temperature of skin
- changes in color or texture of skin

A
- any large or tender lymph nodes
- abnormal or excessive bleeding
- bruising or h/o anemia

A
- any headache or head injury

A
- any ringing in your ears (tinnitus) or hearing loss
- drainage or ear pain

A
- any problem with or loss of sense of smell
- any nasal discharge or nosebleeds (epistaxis)
- any obstruction

A
- any bleeding gums
- any lesions in mouth or gums
- lesions on your lips or tongue

A
- Any sore throats (pharyngitis) or hoarsness
- painful swallowing (odynophagia)
- any post-nasal drip or taste of acid in your mouth or throat
- any reflux

A
- any fullness
- decrease range of motion or pain

A
- Any cough or coughing up blood
- phlegm (sputum) production
- shortness of breath (SOB, dyspnea) or wheezing
- chest pain (CP) when taking a deep breaht (pleuritic)

ROS: Cardiovascular System
A
- any chest pain or palpatations
- light-headedness or passing out (syncope)
- swelling (edema) in your feet
- any h/o heart murmurs or blood clots/phlebitits (DVTs)
- calp pain at rest or with exercise (claudication or pseudoclaudication)
- any SOB with exertion? while lying flat?
- if yes, how many pillows to prevent SOB (orthopnea)
- wake up suddenly SOB
- if yes, need to sit up until it resolves? (paroxysmal noctural dyspnea)
- any change in activity tolerance

ROS: Breast
A
- any pains, lumps or discharge
- do you do regular breast exams

ROS: Gastrointestinal
A
- any change in appetite, feeling of fullness
- distention or abdominal pain
- hearburn (reflux, acid indigestion)
- nausea, vomiting
- vomiting blood (hemataemesis) or coffe ground material
- diarrhea or constipation
- hernias
- yllowing of skin or eyes (jaundice/icterus)
- any recent changes in bowel habits (frequency, consistency or color)
- black or tarry stools
- maroon colored or blood in stool
- pain with bowel movements or hemorrhoids

ROS: Urinary System
A
- any pain on urination (dysuria) or blood in urine (hematuria)
- need to go often [with little volume] (frequency)
- sudden, compelling urge to urinate (urgency)
- increase in volume (polyuria) or make little or no urine (oliguria)
- unable to void (retention)
- dribbling, difficulty getting the stream going (hesitancy)
- voiding multiple times at night (nocturia)
- how many times?
- leaking urine (incontinence)
- tenderness in back or flank (CVA)
- h/o bladder, urinary tract infection (UTI) or stones (nephrolithiasis)

A
- any history of or exposure to sexually transmitted diseases or infection
- any genital lesion or rahs
- pain or discharge (vaginal)
- satisifed with sex life
