56 year old male with history of tobacco use, presented 3 weeks ago with fever, cough and sputum (Image A). Patient was non-toxic appearing and saturations were 96%
Appropriate initial treatment would have included:
- a)Keflex 500mg T.I.D for 7 days
- b)Steroid Inhaler (Pulmicort) for 7 days and home IV Vancomycin
- c)Levofloxacin 750 mg PO q 24 for 7 days as long as afebrile for greater than 48 hours prior to discontinuing
- d)Augmentin 2gm P.O q12 hours for 14 days
