Grading Muscle Strength
- 0/5 = no muscle movement
- 1/5 = visible muscle movement, but no movement at the join
- 2/5 = movement at the joint, but not against gravity
- 3/5 = movement against gravity, but not against added resistance
- 4/5 = movement against resistance, but less than normal
- 5/5 = normal strength
Grading Effusions of Joints
Grade 0-3
- grade 0: no effusion
- grade 1: minimal amount of joint effusion
- grade 2: moderate amount of joint effusion (little distention of the joint capsule)
- grade 3: extensive amount of joint effusion (with high distension of the joint capsule)

Deep Tendon Reflex Grading
0-4
- 0/4 = absent
- ¼ = decreased but present
- 2/4 = normal
- ¾ = brisk, unsustained clonus
- 4/4 = brisk, sustained clonus
Positive Babinski Sign
a positive babinski sign occurs in the absence of descending inhibition
normal in infants, pathological in adults
Clonus
muscular spasm with rhythmic, repeated muscle contractions
Straight Leg Raise Test
tests for sciatica
- While pt is lying supine, raise the patients leg:
- positive test: pain/radicular sxs at 30-70 degrees in the anatomic distribution and has to extend to at least below the knee
- especially positive if the contralateral leg induces pain
- ***dorsiflexion of the foot exacerbates the pain***
Phalen’s Maneuver
only specific for carpal tunnel
Tinel’s Sign
Tap an affected nerve and will reproduce pain → can be done on any superficial nerve but is often done on the median nerve to assess for Carpal Tunnel
Finkelstein Test
pathognomonic for de Quervain’s Tenosynovitis
