Restrictive + obstructive Flashcards
does chest wall disease change slope of PV curve? is DLCO affected?
CWD: no change of PV slope
- gas exchange is limited, so DLCO will change, but not when corrected for alveolar surface compliance
ILD: does have a change in PV slope
What are the FEV1/FVC values in indivs with mixed obstructive/restrictive lung disease?
Must have reduced lung volumes AND FEV1/FVC
Interstitial lung disease:
- what is it?
What is it caused by?
large group of disorders, mostly causing fibrosis of interstitium (the tissue and space around the air sacs of the lungs)
- these are NOT obstructive airway diseases
- collagen vascular diseases
- exposures
- genetic
- idiopathic
pigmented brown macrophages
- what is a difference between the two diseases?
2 smoking related diseases:
Respiratory bronchiolitis,
- mean age is 46
- worsen with glucocorticoids
Desquamative interstitial pneumonia
- smokers in 4th/5th decade of life
- cured with corticosteroids
Bronchiolitis is more common in kids or adults? Cause?
more common in kids, generally RSV, adenovirus, mycoplasma
What disease presents with large quantities of foul smelling sputum?
bronchiectasis
Which disease, bronchiectasis or bronchiolitis affects terminal bronchioles? Which on affects respiratory bronchioles?
Bronchiectasis: terminal bronchioles
Bronchiolitis: respiratory bronchioles
COPD stages
0: at risk I: mild COPD II: Moderate COPD III: Severe COPD IV: Very severe COPD
Characteristics of stage 0 COPD
At risk
Normal spirometry FEV1/FVC ~ 100%
Chronic symptoms
- cough
- sputum production
Characteristics of stage 1 COPD
Mild COPD
FEV1/FVC /= 80% predicted
With or without chronic symptoms
- cough, sputum prod.
Characteristics of stage II COPD
Moderate COPD
FEV1/FVC
Characteristics of stage III COPD
Severe COPD
FEV1/FVC
Characteristics of stage IV COPD
Severe COPD
FEV1/FVC
Mild intermittent symptoms
Mild Persistent symptoms
2x per month
