Causes: miSSHAPEN

A

 Sarcoid

 Systemic sclerosis

 Haemochromatosis

 Amyloidosis

 Primary: endomyocardial fibrosis

 Eosinophilia (Loffler’s eosinophilic endocarditis)

 Neoplasia: carcinoid (→ TR and PS)

A
  • There is restriction to ventricular filling which may result in dependent oedema, ascites and an enlarged liver.

The following points are noted on cardiovascular examination:

  • the JVP is raised
  • Kussmaul’s sign may be detected
  • the apex beat is usually palpable (in contrast to constrictive pericarditis)
  • heart sounds may be distant
  • S3 and S4 may be present

Chest radiograph:

  • mild cardiac enlargement

ECG:

  • low-voltage
  • conduction defects

echocardiogram:

  • increased left ventricular wall thickness
  • systolic function is normal

radionuclide studies:

  • normal systolic function

cardiac catheterization:

  • normal systolic function
  • elevated left- and right-sided filling pressures
