restrictive cardiomyopathy Flashcards
1
Q
define
A
get restrictive fillinh of ventricles [here contractile ie squeeze function of heart = okay, but relaxation/filling of heart is abnormal]
2
Q
Causes: miSSHAPEN
A
Sarcoid
Systemic sclerosis
Haemochromatosis
Amyloidosis
Primary: endomyocardial fibrosis
Eosinophilia (Loffler’s eosinophilic endocarditis)
Neoplasia: carcinoid (→ TR and PS)
3
Q
s/s
A
- There is restriction to ventricular filling which may result in dependent oedema, ascites and an enlarged liver.
The following points are noted on cardiovascular examination:
- the JVP is raised
- Kussmaul’s sign may be detected
- the apex beat is usually palpable (in contrast to constrictive pericarditis)
- heart sounds may be distant
- S3 and S4 may be present
4
Q
ix
A
Chest radiograph:
- mild cardiac enlargement
ECG:
- low-voltage
- conduction defects
echocardiogram:
- increased left ventricular wall thickness
- systolic function is normal
radionuclide studies:
- normal systolic function
cardiac catheterization:
- normal systolic function
- elevated left- and right-sided filling pressures
5
Q
rx
A
Treatment is symptomatic.
