- congenital abnormality (rare)
- complication of surgery to right ventricular outflow tract e.g. tetralogy of Fallot repair
- dilated pulmonary valve ring in severe pulmonary hypertension

A
Right ventricular overload is well tolerated long-term and the condition
is often overshadowed by the associated cardiac or pulmonary vascular disease.

Differentiation from aortic incompetence
A
- there is no collapsing peripheral pulse.
- loud P2 and right ventricular hypertrophy.
- if there are no peripheral signs of aortic regurgitation or pulmonary hypertension then the murmur is almost always due to mild aortic regurgitation.

valve replacement (homograft or xenograft).
heart and lung transplantation - if there is irreversible pulmonary hypertension.
