Reports and Returns Flashcards
What’s the difference between reports and returns
Returns are documents which are prepared and submitted as part of the material maintenance process
Where as
Reports and prepared and submitted to demonstrate progress with maintenance process.
What is a change proposal
A proposal by any authorised organisation to modify or dispose of material
Where must all defects be recorded
In AMPs
What does an URDEF report
An URDEF is a Capability related defect
When are URDEFs raised
When a unit cannot meet the specified and implied tasks of a mission due to a material deficiency.
What are the main characteristics of an URDEF?
a. Priority
b. Capability Impact Statement
c. Defect description
d. Engineering assessment
e. Engineering and logistics support required
What is the priority levels of URDEFs
- PRI ONE SAFE
- PRI ONE OPS
- PRI TWO
- PRI THREE
What is a PRI ONE Safe defect?
A safety related defect, condition deficiency that precludes a ship from remaining at sea or sailing
What is a PRI ONE OPS URDEF
A defect or deficiency that prevents the ship from completing a specified or implied task.
What is a PRI TWO defect
A defect or condition that significantly limits seaworthiness, personal safety or operational capability but does not preclude it from scheduled operational activities.
What is a priority 3 defect?
A defect or condition that does warrant classification as priority one or two because an alternative solution or redundancy exists.
What is capability impact statement
- a statement of system loss
- an assessment of impact to the units capability
When completing an URDEF, what information is included in the “Defect description”
- the facility code
- a concise description of defect and what further investigation is required
What is the engineering assessment in an URDEF
Normally completed by HOD, and is to identify the cause, and alternate solutions and temporary changes to authorised configuration baseline
When compiling and URDEF, what information is included in the ‘Engineering and Logistics Support’ section
- stores demand
- reference no & tm200
- assistance required
- specialist tools
- ship availability
What is the precedence of URDEF Signals
PRI One Safe/Ops - immediate
PRI Two - priority
PRI Three - Routine
When is an URDEF cancelled
When an URDEF is reported incorrectly
What is to be included in the rectification signal
- results from STW and testing conducted
- information and knowledge gained
- effects on supporting systems
- other system or equipment details
What is a TM200
TM200 is a maintenance control record. Normally used to request external support for unplanned maintenance. It must be used to return material to approved configuration baselines.
What would you use a TM200 used for?
External maintenance assistance such as
a. Ship husbandry
b. Service material
c. Diagnose material
d. Repair material
What information is required for a TM200
a. Location
b. Job Title
c. Description
d. Amps no.
e. CMC
f. Resources required
g. A statement of work
h. Repair method.
How long is the usual maintenance response for a TM200
PRI 1 or 2 URDEF: ASAP
Pri 3 or 4: Normally next maintenance period
Why is non-conformance reporting important
- informs maintenance planning and costing
- highlights design deficiencies
- provides a basis for spare parts holdings
- provides information about the refinement of the Material seaworthiness reference set
- informs decisions wrt acquisition of new equipment
What is the purpose of raising an RPR
- Identify a material problem
- Propose new material or capability
- Propose a change or modification
- Request a trial
What is the form number for an RPR
AD514
What is the form number for a Change Directive
AD514-1
What is the form number for a CIR
Change Implementation Report: AD514-2
What is a variance
It’s an engineering change that is a departure from the approved baseline for a limited time for a specified purpose if it does not require a revision of the approved product
What is a major change
A change that:
- impacts form, fit or function
- has the potential of adverse functions
- has significant costs
- impact on certification
- requires retrofit
- requires operator training
What are the typical priority levels for ECs:
Emergency
Priority
Routine
What is the DAR
The Design Acceptance Representative maybe be involved in the change process. These are warrant holders who’s responsible to conduct validation of design, design process and OQE and identify the risk to technical integrity associated with design.
