What characteristics should a physical isolation device have

A

a. Identifiable. Any person who sees a lockout device must be able to recognise who applied the lockout and understand its purpose; all isolations/lockouts must be tagged.
b. Durable. Lockout devices must be suitable for the environmental conditions in which they are used, (wet, damp, vibratory or corrosive conditions). In particular, it must be able to withstand conditions that could lead to the isolation failing; for example, attached to equipment which is vibrating.
c. Substantial. Lockout devices must be strong enough that they cannot be easily tampered with or removed inadvertently.
d. Exclusive operation. Where a physical key lock is applied, each lock requires a unique key or combination.

What does the IO do?

A

The IO holds the supervisory role to check the complied RFI and approve it for use. They can perform all the roles of the IW. They must vet RFI’s for correct details and ensure the correct isolation points are detailed on the RFI.
The IO is to ensure the work area is safe to restore the plant. Once satisfied, they issue the key for the IW to restore the plant.

What is the procedure if a Personal Isolation Lock is not removed?

A

The Job Controller and Isolation staff must attempt to contact the owner of the lock and recall them.

If the the person can be contacted and it’s not practical to return, the locker owner will be informed that their lock will be removed. The isolations alteration record document the time, date and approval for lock removal. The personal lock is the be removed by the isolations officer.

If the personal lock cannot be contacted, the IW and IO must conduct a thorough inspection of the work are to ensure no person is in the position of danger. The IO will contact the appropriate authority or principle contractor to gain permission to remove lock. This is recited in Isolation Alteration of the RFI and signed by IO, EO and principle RA. A

IAS can perform tasks of the IO an IE, however they are employed within the system to conduct audits, Ship’s Master Isolation Dataset and template maintenance and ensure the Safe Systems of Work and authorisation matrix are maintained correctly.

What occurs when an individual loses the key to the Personal Isolation Lock
In the event that an individual loses the key to their Personal Isolation Lock, the lock may be cut off by the Personal Isolation Lock owner in the presence of an IW. This event is to be recorded in section 14 on the RFI. A new Personal Isolation Lock will be issued and recorded and the new Personal Lock will be applied before any work can recommence.

What procedure is to occur when you are not able to apply an isolation device?
In circumstances where Isolation devices are not practicable due to equipment configuration, or the potential for fitment of a lockout device to introduce additional hazards and/or risks, an isolation assessment of the system/equipment in the form of a SRA must be undertaken to determine how to effectively isolate the equipment/system SFARP. In such instances authorisation must be sought from the EO to proceed.

When is the ships isolation lock used
1. Ship’s Isolation Locks are to be used to secure the physical lockout device unless the isolation is utilising a Single Restricted Isolator/Isolation process. Ship’s Isolation locks are only to be applied to and removed from isolation devices by an IW as authorised in the platform’s authorisation matrix. 2. Ship’s isolation locks are also utilised as the Primary Isolation Lock used to lock the lockbox containing all the keys and tag stubs of the ship’s isolation locks used on the isolation devices for each RFI. Lockbox primary lock keys are to be placed onto the Ship’s Isolation Keyboard immediately after the lock has been applied. This ensures the keys are secured and controlled by an Isolation Officer (IO).
