Which stones are radiopaque?
Anything with minerals:
- calcium oxalate
- calcium phosphate
- Mag-Ammonium-Phos (struvite)
What do WBC casts indicate?
Tubular disease (e.g., acute tubular necrosis)
Patient with nephrotic syndrome develops fever and abdominal tenderness
Spontaneous bacterial peritonitis (likely S. pneumoniae)
Testicular pain relieved by lifting testicle
Epididymitis
Sexually active female with pleuritic right upper quadrant pain
Fitz-Hugh-Curtis syndrome (gonorrhea)
Triad: Thrombocytopenia, microangiopathic hemolytic anemia, and acute kidney injury
Hemolytic-uremic syndrome
- Bonus: commonly a/w Shiga toxin producing E. coli (STEC) infections such as E. Coli O157:H7
Patient with fever and rash who recently started new medication.
Labs:
- increased serum creatinine and eosinophils
- UA with WBC, WBC casts and eosinophils.
Diagnosis?
acute interstitial nephritis (AIN)
Electrolyte abnormalities in rhabdomyolysis: Ca, K, Phos
Low Ca
High K
High Phos
