what is sevelamer? What is it used for? When is it contraindicated?

A

A phosphate binder. Binds phosphates in the gut to reduce absorption of phosphates. This is used in chronic renal failure where you have hyperphosphatemia. Contraindicated in bowel obstruction

how do we manage anaemia of chronic renal failure?

A

use a synthetic EPO such as Darbepoetin alfa. Contraindicated in uncontrolled hypertension

3 drugs that can lead to acute renal failure- aka the triple whammy?

A

NSAIDs –> reduce prostaglandin production required to dilate afferent arteriole
ACE inhibitor –> reduce Ang2 production required to constrict efferent arteriole
