what is an immediate denture?
a denture inserted on the same day as the tooth or teeth are removed
can be partial or complete
what are the advantages of immediate dentures?
aesthetics
speech
maintain function
avoid drifting/ tilting of remaining teeth

what are the disadvantages of immediate dentures?
a space is likely to develop between the denture and ridge as healing occurs
may need replaced after initial resorption - creating an additional cost
what are the clinical stages of immediate dentures?
- exam and assess
- primary impressions

in creating an immediate denture, what does the exam and assess stage involve?
assess what teeth have to be removed clinically and radiographically if required
in creating an immediate denture, what type of primary impressions are taken?
alginate in stick trays - ensure accurate and fully extended
when may you not need to record occlusion for an immediate denture?
if sufficient teeth present to hand articulate the casts - anterior and posterior index teeth
what is important when taking 2nd imps for immediate dentures?
ensuring it is fully extended around the areas of teeth to be XLA
what is performed at the try in stage for an immediate denture?
check extensions
check occlusion
check shade and alignment of teeth
specifically mark on the casts what teeth will be extracted
why should you avoid re trying the immediate denture in at finish?
you need to be careful not to dislodge the blood clot
what are the instructions for the lab for an immediate denture?
which teeth to be XLA
arrangement of teeth

date for denture to be ready for insertion
what are the types of flanges that could be used for immediate dentures?
full flange - engages the entire sulcus depth
part flange - engages just part of the sulcus depth
flangeless - tooth sits on the edentulous ridge
when may you not be able to provide a full flange?
too big an undercut on the ridge
what must you do with immediate denture fit prior to inserting local for xla?
check the fit of the denture
what may the immediate denture feel like to the patient initially?
bulky due to LA or the transition to a larger denture
what are the post op instructions given for immediate dentures?
xla post op
keep dentures in for 24hours
after 24hours - warm saline mouthwash and remove dentures at mealtimes
clean denture with soft toothbrush, soap and water
when do you review a patient after immediate denture?
the day after insertion
after 1 week
after 1 month
why do you tell the patient to keep immediate denture in for 24 hours after insertion?
if the denture is removed, the tissue swelling wont allow the denture to fit back in
why do we review an immediate denture 24 hours after insertion?
at the time of insertion, the patient is numb so wont feel if anything is uncomfortable
to identify any areas of inflammation to ease
in the 1 month review for an immediate denture what do we assess?
adaptation
at what point after immediate denture insertion, may you consider a new denture?
at the 6 month recall
may you always need another denture after an immediate denture?
no
sometimes only a rebase for full
what must patients be warned off with the transition from partly dentate to edentulous?
ridge resorption and the need for early rebasing/ replacement
what treatment can be provided to increase the chance of successful adaptation to F/F, particularly for older pts?
a gradual transition via additions to P/P
