Define IUGR
Slower than normal rate of growth. a condition in which fetus is unable to achieve it genetically determined potential size due to pathophysiology. typically <2.5kg and/or below 10th centile
How to identify antenatally?
symphysis fundal height should be plotted on a customised chart
single SFH< 10th ceentile or measurements showing static growth refer for USS
What does a USS measure ?
Fetal abdominal circumference and EFW <10th centile diagnose SGA
If diagnosed SGA?
offer serial doppler
Symmetrical IUGR
begins in first trimester
Caused by combination of intrinsic and extrinsic factors
intrinisic- assocciated with infection/chromosomal factors
extrinsic- lifestyle, domestic abuse
problem associated with symmetrical
stillbirth
chromosonal abnormalities
infection
Asymmetrical
3rd trimester - growth normal in 1/2nd
large head
extrinsic factors that disrupt placental perfusion
problems with asymmetric
stillbirth mec aspiration hypoxia nec prem olihydramnios
olihydramnios
300-500mls
small abdo
less intrauterine space
