Missing Offender File

A

Make every effort to locate the file. Check with other staff via email or in person, look in all file cabinets and drawers, and mail and file rooms. If you cannot locate the missing file, notify your supervisor and complete an Incident Report

You may also print additional offender information and documents from OBIS. Obtain the required signatures on the appropriate forms, such as the Order of Supervision, Individualized Supervision Plan (ISP), and OFOA.

After gathering all relevant information, documents, and signatures relating to the missing case file, you are now ready to recreate the file

To recreate a missing offender case file, obtain copies of the official documents related to an offender from the appropriate agencies or entities. Relevant documents may include:

• Order(s) of Supervision
• violation report(s)
• affidavit(s)
• warrant(s)
• relevant arrest report(s)
• Presentence Investigation (PSI)
• any other written record relevant to the supervision or investigation

Contact the appropriate entities or agencies by phone, facsimile, email, standard mail, or personal visit to obtain the missing documents. Agencies and resources to contact include:

• clerk of the court
• court or sentencing authority
• law enforcement agencies
• office of the state attorney
• treatment or service providers
• any other relevant entity or department that may be involved in the supervision of an offender
• DC intranet
• DAVID
• Bureau of Classification—a section within the Florida Department of Corrections responsible for the management of inmates while incarcerated, that assists inmates with successful re-entry into the community upon release
• Comprehensive Case Information System (CCIS)—portal used by officers to access clerk of court information and records throughout Florida

A

Organize the gathered replacement information and documents, using an empty six-part or two-part file, and follow the Department-approved Offender File Organization Sheet guidelines. Document all actions taken in recreating a missing offender case file in OBIS following Department policy.
