What isnt in FFP
Factor 5, 8 and platelets. Has all other factors
Barretts - what cell types, Prague classification, Seattle protocol, tx and surveillance
Replacement of squamous with columnar (metaplasia)
BE high grade dysplasia 6-12% annual cancer incidence
Prague classification - measure from cardia fold to proximal most edge of salmon-colored tissue, as visualized with stand white light and chromoendoscopy
EMR = diagnosis and treatment, good for short non-circumferential
RFA depth of 0.5mm
First get tissue diagnosis via Seattle: 4 quadrants q1 cm from GEJ to normal tissue
Second Tx and surveillance:
- HGD or nodule –> EMR nodule + RFA to Barrett’s area –> repeat EGD at 2 months and RFA again if not resolution, repeat q2-3m til resolved
LG dysplasia –> repeat EGD after PPI bid x8-12 weeks, consider RFA –> EGD 6m and 1y
No dysplasia → EGD 3-5y
