Paraneoplastic - Cushing syndrome: what cancer, symptoms, how test and treat
small cell
bronchial and thymic carcinoids.
ACTH
edema, weakness, HTN, hyperglycemia, hypoK, osteoporosis, hirsutism, confusion.
Test: dexamethasone suppression (wont suppress ACTH), OCTREOTIDE SCAN for other tumors/mets.
Tx: steroid and ADH blockers
Paraneoplastic - muscle weakness and rashes on hands/under tongue
Dematomyositis/polymyositis - Adenocarcinoma
Paraneoplastic - hypercalcemia

Paraneoplastic - Hypertrophic pulmonary osteoarthropathy
Adenocarcinoma. Proliferation of tubular bones causing joint pain with PET avidity. NSAIDs
Paraneoplastic - Lambert-Eaton. Cancer, pathophys, sx, tx
Small cell.
Antibodies to pre-synaptic voltage-gated Ca channels (ACh release). Proximal muscle weakness. Tx 3,4-diaminopyridine
Paraneoplastic - SIADH. Cancer, sx, tx
Small cell. AMS, hyponatremia, anorexia and nausea. More common in women. Hypertonic saline, vasopressin receptor antagonists
Paraneoplastic - flushing, diarrhea, bronchoconstriction
Carcinoid syndrome from metastatic carcinoid tumors. Flushing, diarrhea, bronchoconstriction. Serotonin. Measure 5-HIAA in urine or serum chromogranin A.
Treatment for superior sulcus adenoca
Invasive mediastinal staging first is must. Then neoadjuvant chemoRT –> lobectomy and reconstruction of vessels as necessary. Sx from T1 nerve root
