1st line: CCB, endothelia receptor antagonist or phosphodiesterase - 5 inhibitor
+ loop diuretic (furosemide)
+ anticoagulant (warfarin)
+ digoxin
+ supplementary oxygen
what test is diagnostic for pulmonary hypertension?
right heart catheterisation
what criteria is diagnostic for pulmonary hypertension?
mean pulmonary arterial pressure > 25mmHg
pulmonary capillary wedge pressure < 15mmHg
pulmonary vascular resistance > 3 wood units
what are the causes of pulmonary hypertension?
idiopathic LVF mitral regurgitation chronic thromboembolic pulmonary hypertension chronic respiratory disease e.g. cor pulmonale in COPD congenital heart disease e.g. VSD collagen vascular disease portal hypertension
what signs on examination might you illicit in a patient with pulmonary hypertension?
loud pulmonary component of the 2nd heart sound (S2)
tricuspid regurgitation murmur (halo systolic murmur heard in left sternal border)
