what features from thoracentesis supports diagnosis of empyema?
cloudy = complicated effusion, frank pus = empyema
LDH > 2-3x upper limit normal PH < 7.3 glucose < 3.3 protein > 30 white cell differentiation: polymorphonuclear leukocytes
what is the management for an empyema?
IV antibiotics + chest drain
+/- fluid resuscitation
+ antipyretics and analgesia
describe the follow up after an empyema.
al patients receive follow up for WCC and CRP 2-3 weeks after discharge
all receive a CXR 4-6 weeks after discharge
if restrictive pulmonary functioning, it usually resolves within 6 months. if not then surgical decortication
what feature on CXR is suggestive of empyema?
pleurally based D shape
