Pulmonary CIrculation Flashcards

1
Q

Describe the pulmonary circulation.

A

Largest vascular bed
HIGHLY COMPLIANT vessels
accomodates about the entire Cardiac Output
Pulmonary artery - larger, shorter, more thin walled

3
Q

Describe the Broncial circulation.

A

1-2% Cardiac output
Empties into the pulmonary veins
Generates the physiological shunt

Q

What happens in Hypoxic pulmonary Vasoconstriction?

A

Decreased alveolar PO2 causes local vasoconstriction, which leads to blood redistribution to better aerated alveoli.
Defense mechanism to help survive, Unique to lungs
We want to match PO2 to perfusion

Q

What is the common trigger of the HPV response?

Hypoxic Pulmonary Vasoconstriction

A

Alveolar Hypoxia

Chronically is the main cause of Pulmonary Arterial Hypertension

6
Q

What is the effect of gravity on Pulmonary blood flow?

A

blood flow is greater at the base.

Pulmonary circ is a low pressure/low resistance system

What are the factors that determine the pulmonary capillary flow?

A

Intravascular Hydrostatic pressue, IV oncotic pressue, Capillary permeability, Interstitial Hydrostatic pressue, IS oncotic pressure, lymphatic drainage, alveolar membrane permeability.

13
Q

Is the Ventilation/Perfusion ratio higher or lower in the apex as compared to the base?

A

Higher.
The Perfusion fall faster than the ventilation
Therefor the ratio increases while both values decrease

15
Q

What is the V/Q ratio in dead space?

A

infinity
alveolar gas has the same composition as humidified inspired air
ex: pulmonary embolism

16
Q

What are some right-left shunts?

A

Atrial/Ventricular Septal Defects
Hypoxemia always results, bypasses lungs
could also be due to obstruction of one of the airways

17
Q

In pulmonary embolism, does the pressure increase in right or left heart?

A

Right

Causes Right to left shunting and V/Q mismatch

18
Q

How do you diagnose Pulmonary Embolism?

A

CT scan, PULMONARY ANGIOGRAM, High Probability ventilation (Ventilation scan will not match the Perfusion scan)
