What are the features of baby blues?
Transient self-limited state characterised by; tearfulness, labile mood, irritability and anxiety
Onset 2-3 days after delivery
Subsiding over a period of 1-5 days
What is the management of baby blues?
Supportive care, including validation of feelings and an explanation of Sx
Follow up R/V to make sure they have settled
Ddx for perinatal mental health
Baby blues
Postnatal depression
Postnatal psychosis
What are the risk factors for postnatal depression?
PHx depression Antenatal depression Multiple stressors in last year Few social supports Quality of marital relationship Childhood sexual assault Poor early relationships Personality factors Obstetric issues e.g. difficulty getting pregnant Socioeconomic factors Thyroid dysfunction Anaemia Unwanted pregnancy Colicky or sick child
When does postnatal depression occur?
Usually 2-4/52 postpartum
What test should you do for postnatal depression?
Edinburgh Postnatal Depression Scale
What is the management for postnatal depression?
Social supports/ groups
Psychotherapy
Aantidepressants
Referral to mental health professional if severe
What are some complications of perinatal depression?
Poor antenatal attendance and self care Low birth weight Small head circumference Preterm delivery Suicide/ infanticide
What are the features of postpartum psychosis?
Very rapidly evolving, typically 1-2 weeks postpartum
Can be new onset or relapse (especially if Hx of bipolar)
What are the Sx of postnatal depression?
Sx typically persist for at least 2 weeks Persistent sadness/ empty mood Guilt Fatigue Anhedonia Low libido Changes in appetite Poor self care Social withdrawal Sleep disturbance Trouble bonding with the baby Feeling inadequate in taking care of the baby Worry about harming self, baby or partner
