Delegation Criteria
A
- it must be a routine act of living
- the client must be in stable condition
- the outcomes must be known and predictable
- the PSW must receive the appropriate, client specific training
- a PSW must be supervised and monitored before completing a delegated task alone
- there must be support available if you require it each time you complete a delegated task. This support could be over the phone

Documenting Rules
A
- if handwritten, must be in ink, no pencils allowed
- be clear and concise, including objective observations. Facts only, no opinions
- no white space, draw a line through it
- each entry must have the client’s name on the page, the date, the time and the signature of the writer
- no white out allowed
- documentation must be accurate. Documenting as soon as possible after an event maximizes accuracy
- Document things you observed yourself, not what other staff have told you occurred
