Psoriasis/Acne Flashcards
What are the TNF-alpha antibodies that treat psoriasis?
Adalimumab, Etanercept, Infliximab
Infliximab ADE
- Heart failure
2. Murine protein hypersensitivity
Etanercept ADE
Sepsis
BBW for all TNF-alpha antibodies?
Infection, neoplasia, TB
Ustekinumab MOA
binds p40 subunit integral to IL-12 & IL-23 (over-expressed in psoriasis) –> suppression of Th1 & Th17 cells
Ustekinumab ADE
Increased risk of infection & neoplasia; live virus vaccinations are contraindicated. Test for TB before initiating infection
Which retinoids target retinoic acid receptors (RARs)?
Tretinoin, Adapalene, Tazarotene
Acute ADEs of retinoids
Dry skin, epistaxis, conjunctivitis, reduced night vision, alopecia
Before starting therapy with retinoids, what must you do?
Baseline serum lipid, serum transaminases, CBC and pregnancy test (RETINOIDS ARE TERATOGENS!!)
Isoretinoin (accutane) serious ADE?
Depression, suicidal ideation
Calcipotriene MOA
Vitamin D analog –> treats psoriasis
Calcipotriene ADE
- Topical irritation
- Hypercalcemia
- Increased susceptibility to UV induced skin cancer
Calcitriol MOA
Hormonally active form of Vitamin D
Calcitriol difference from Calcipotriene?
Better tolerated
Benzoyl peroxide MOA
Prodrug –> converted to benzoic acid –> free radical liberation lethal for nearby P. acnes
