Mx of warfarin in major bleed
A
- Stop warfarin
- Give 5-10mg IV vitamin K
- Give prothrombin complex

<5 - reduce dose
5-8 - omit warfarin for 2 days then reduce dose
>8 - omit warfarin and give 5mg oral vitamin K
