How much fluid to give in a bolus?
20ml/kg except in diabetes or trauma, then give 10ml/kg
What to give for maintenance fluids?
0.9% saline + 5% dextrose 500ml + 10mmol potassium chloride
Which abx to give for ?sepsis?
Ceftriaxone
What to give if they have a high temp?
Paracetamol PRN
Why is the lactate higher in venous blood?
Indicates anaerobic respiration
How to assess dehydration?
Does the child: appear well has altered responsiveness – for example, is irritable or lethargic has decreased urine output has pale or mottled skin has cold extremities
What signs would make you think of hypernatraemic dehydration?
jittery movements increased muscle tone hyperreflexia convulsions drowsiness or coma
When to use oral rehydration solution + how much?
Give 50 ml/kg low-osmolarity ORS solution over 4 hours, plus ORS solution for maintenance unless IVF indicated
Continue breastfeeding.
When are IV fluids indicated?
If there is shock or suspected shock
Signs of clinical dehydration
Altered responsiveness Appear to be unwell Sunken eyes Dry mucus membranes Tachypnoea Tachycardia Reduced skin turgor Decreased urine output
Signs of hypovolaemic shock
Same as clinical dehydration AND: Decreased consciousness Hypotension Weak pulses Prolonged CRT Pale/ mottled skin Cold extremities
How to work out flow rate for infusion?
Flow rate = Volume (ml) / Time (hours)
