What is prostatitis?
Inflammation of the prostate gland
2 types:
• bacterial
• abacterial (chronic pelvic pain)
Is bacterial prostatitis caused by gram positive or gram negative?
Gram negative
E.g. E.coli
Are leukocytes found in urine in abacterial prostatitis?
Yes
But no pathogens
Symptoms of acute and chronic prostatitis?
Acute:
• dysuria
• increased frequency of urinating
Chronic:
• pelvic pain
• milder symptoms than acute
Treatment of acute and chronic prostatitis?
Acute bacterial prostatitis:
Quinolone
Chronic abacterial prostatitis:
Alpha blocker & antibiotic
All patients:
• Paracetamol/ibuprofen
• laxative (if stool is pressing on rectum)
Quinolone MOA
Inhibits activity of DNA & Topoisiomerase
What is Erectile dysfunction?
Inability to get or main an erection, that is enough for sexual intercourse
What are the Contributing physiological & contributing factors?
Physiological
• depression
• anxiety
• excess alcohol
• history of psychological abuse
Drugs:
• marijuana
• methadone
• alcohol
• cocaine
Pharmacological options ?
Phosphodiesterase type-inhibitors (type 5)
MOA: increases blood flow to penis
Examples:
• Avanafil
• tadalafil - longer acting
• slidenafil
Second line?
Alprostadil
MOA: prostaglandin E1analogue
OTC Medicine ?
Slidenafil - viagra connect
Tadalafil
Premature ejaculation?
Brief sextual latency, loss of control and physiological distress
Pharmacological treatment?
Dapoxetine
MOA: SSRI
