How to confirm PROM

A

History of watery clear fluid PV
Urine dipstick shows fluid in vagina is alkaline
Speculum exam shows pooling of clear fluid
Vaginal fluid positive for IGF1 or alpha-microglobulin 1

A

Intrauterine infection
PPH due to retained placenta
APH due to placental abruption
Increased need for intervention

A

Intrauterine infection

Umbilical cord prolapse = hypoxia

Management of PROM

A

Erythromycin 250mg QDS for 10 days or until labour starts (whichever first)
Blood test - FBC, CRP
Monitor fetal HR
Either wait maximum 24 hours for spontaneous labour OR induce
