Symptoms of PID
A
Lower abdo pain +/- RUQ pain Dyspareunia Vaginal discharge IM or PC bleeding Fever

Complications of PID
A
Tubal disease leading to infertility 10% Ectopic pregnancy risk increased by x10 Chronic pelvic pain/chronic PID (adhesions) Ovarian abscess May require surgical intervention

CDC diagnosis criteria for PID
A
Major criteria:
Lower abdo pain
Cervical motion or adnexal tenderness on examination
Minor criteria: Fever Raised inflammatory markers Cervical discharge Swab positive for chlamydia or gonorrhoea
Confirmed by laparoscopic findings

A
Pregnancy test STI screen Inflammatory markers Urine dipstick USS Laparoscopy

Treatment
A
Doxycycline 100mg BD for 2 weeks
Ceftriaxone 500mg IM stat
Metronidazole 400mg BD for 2 weeks

When to admit PID to hospital
A
Acutely unwell (baseline obs) Not tolerating or responding to oral ABx after 48 hours Tubo-ovarian abscess Ovarian torsion Pregnancy Peritonitis (any guarding)
