Causes of gradual visual loss
Refractive errors, cataracts, age-related macular degeneration, primary open angle glaucoma and diabetic maculopathy
Causes of red eye
Subconjunctival haemorrhage, dry eyes, conjunctivitis, corneal abrasion, keratitis
Common causes of acute visual loss
AACG, CRAO, vitreous haemorrhage, CVA/TIA
Causes of flashes and floaters
Posterior vitreous detachment, migraine with aura
Causes of abnormal eyelids
Blepharitis, chalazion, BCC, herpes zoster ophthalmicus, entropion, ectropion
Causes of double vision
Strabismus and blowout fracture
Causes of irregular pupils
Horner’s syndrome, Adie’s pupil
Painful causes of red eye
AACG, endophthalmitis, anterior uveitis, scleritis, corneal abrasion, keratitis
Painless causes of red eye
\subconjunctival haemorrhage, dry eyes, conjunctivitis, episcleritis
What are the ocular emergencies
AACG, cranial nerve III palsy, orbital cellulitis, temporal arteritis/GCA, endophthalmitis, absent red reflex in child, retinal detachment, central retinal artery or vein occlusion, trauma
Systemic diseases which commonly affect the eye
Diabetic retinopathy, thyroid eye disease, HTN retinopathy, Uveitis, Sjogren’s syndrome, scleritsis and episcleritis
Rarer causes of acute visual loss
Temporal arteritis, retinal detachment, CRVO, optic neuritis, anterior ischaemic optic neuropathy
Important history to elicit
Vision loss or blurring, onset and timing, partial or complete loss, associated with headache, painful or painless, photophobia, double vision
Past ocular history important to ask
Glasses prescriptions, contact lens, recurrent eye surgery, recent or past diagnosis, amblyopia
Examination of the eyes
Snellen visual acuity, pupils, gross exam, gross visual fields, slit lamp
