CDC Clinical Eligibility for PrEP
Documented negative HIV test before prescribing, no signs/symptoms of acute HIV, normal renal function, no CI’ed meds, documented HBV infection/vaccination status
Treatment for PrEP
Truvada (FTC/TDF 200mg/300mg) PO QD for up to 90 days
Other services for PrEP (lab monitoring)
Assess renal function at 3 and 8 months, test for bacterial STIs every 3-6 months
Indications of PrEP use by MSM: 4 requirements
Adult male or adolescent weighing >35kg
No acute or established HIV infection
Any male sex partner in the last 6 months
Not in a monogamous relationship with a recently tested, HIV-negative man
Indications of PrEP use by MSM: they have to meet at least 1 of these 3 criteria
Any anal without a condom in the last 6 months
Bacterial STIs in the last 6 months
High number of sex partners/commercial sex worker
Indications of PrEP use by heterosexual men and women: 4 requirements
Adult or adolescent weighing >35kg
No acute or established HIV infection
Any sex with the opposite sex partner in the last 6 months
Not in a monogamous relationship with a recently tested, HIV-negative partner
Indications of PrEP use by heterosexual men and women: they have to meet at least one of these criteria
-A bisexual male
-Infrequent condom use with ≥1 partner(s) with unknown HIV status at substantial risk of HIV infection
-An ongoing relationship with an HIV+ partner
-Bacterial STI in the last 6 months
-High number of sex partners/commercial sex worker
In high HIV prevalence area or network
Indications for PrEP use by people who inject drugs: 3 requirements
Adult or adolescent weighing >35kg
No acute or established HIV infection
Any injection of illegal drugs in the part 6 months
indications of PrEP use by people who inject drugs: must meet at least 1 of these criteria
Any sharing of injection or drug preparation equipment in the past 6 months
Risk of sexual acquisition
Truvada prescribing points
Provide script or refill authorization for no more than 90 days
Patients MUST be reassessed every 90 days for continued use
Can be taken indefinitely as long as the individual continues to be at risk for HIV infection and isn’t experiencing ADEs
Truvada ADEs
Diarrhea, nausea, fatigue, headache, dizziness, depression, insomnia, abnormal dreams, rash, abdominal pain, weight loss
Importance of adherence to Truvada
The more adherent a patient is, the better it will work
